Comprehensive Care of Oakland
Subacute care programs serve patients who require more intensive care than what is available in a traditional skilled nursing facility. Subacute care programs can therefore alleviate the high demand for hospitals and the expensive bed availability, and are a better way to serve patients along the continuum of care at a more affordable location.
Skilled nursing facilities that provide subacute care are currently in high demand and the need will continue to rise as the elderly population increases. The facility will provide 24-hour licensed nursing care from stable, consistent, caring staff. Patients are admitted from the acute care hospital to the subacute unit following significant medical events such as cardiac arrest, acute respiratory failure, cerebral vascular accidents, or other neurological events that have led to dependence upon an artificial airway (tracheostomy) and often mechanical support for breathing (ventilator) 24 hours a day. Such patients require either a short-term (temporary) period of rehabilitation or long-term care.
The SFRC skilled nursing facility, Comprehensive Care of Oakland (CCOO) is licensed by the California Department of Public Health and for Medi-Cal and Medicare reimbursement for subacute skilled nursing care. CCOO is partnered with Kaiser Permanente, one of the largest healthcare providers worldwide, to receive patients referred by physicians for subacute care.
